Botulinum toxin for hyperhidrosis – a minimally invasive option for excessive sweating

Hyperhidrosis: A discreet treatment option with botulinum toxin for excessive sweating

Sweating serves an important function in the human body: it regulates body temperature and helps prevent overheating in warm weather or during physical exertion. However, in some people, this mechanism becomes unbalanced: even light physical activity, stressful situations, or even rest can lead to excessive sweating in the armpits, hands, feet, or other areas of the body. This form of excessive sweating is known as primary hyperhidrosis. For those affected, it is more than just a cosmetic problem: clothing becomes constantly soaked, shaking hands becomes difficult, and social well-being suffers.

At the private practice Schoenheits-Hand-Werk in Wallerfangen Dr. med. Gunther Oldag offers targeted, minimally invasive treatment with botulinum toxin, with the aim of noticeably reducing sweat production in the affected areas for several months.

What is hyperhidrosis?

Hyperhidrosis describes excessive sweating that cannot be explained by external factors such as heat or physical exertion. It usually occurs symmetrically, for example, in both armpits, both palms, or soles of the feet. The sweating occurs regardless of temperature or activity level and can significantly impair daily life.

A distinction is made between primary hyperhidrosis (without a recognizable physical cause) and secondary hyperhidrosis (as a result of other diseases or medication use).

How does botulinum toxin work in cases of excessive sweating?

Botulinum toxin is a biologically produced protein that specifically blocks signal transmission between nerves and glands or muscles. In the treatment of hyperhidrosis, the toxin's property of inhibiting the release of the neurotransmitter acetylcholine is utilized. This messenger substance is responsible for activating the sweat glands.

By injecting botulinum toxin into the affected skin areas, the activity of the sweat glands is significantly reduced without permanently destroying them. The effect begins after about four days, with the maximum achievable result visible after approximately four weeks. The skin remains dry, and sweating is noticeably reduced – the effect typically lasts four months, depending on individual activity level, metabolism, and the treatment area.

In which cases is treatment recommended?

Botulinum toxin therapy for hyperhidrosis may be beneficial if:

  • conventional measures such as antiperspirants are insufficient,
  • sweating is socially or professionally stressful,
  • Clothing is regularly soaked with sweat,
  • medical causes were ruled out
  • surgical intervention (e.g., sweat gland removal) is not desired.

Before any treatment, we take the time at our aesthetic medicine and surgery practice for a thorough medical history and consultation. During this process, we determine whether the prerequisites for botulinum toxin therapy are met and whether alternative measures should be considered.

Which regions can be treated?

In our practice, we primarily treat the following areas:

  • Armpits (axillary hyperhidrosis)
  • palms
  • soles of the feet (plantar)

Other areas (e.g., forehead or back) are also possible in principle, but must be assessed individually. Treatment in the underarm area has proven particularly effective, as a significant effect can be achieved with relatively little active ingredient – with good tolerability and suitability for everyday life.

Whether you are often the center of attention at work, suffer from sweaty hands in everyday life, or simply want to wear clothes with confidence again – we advise you openly and honestly.

Treatment procedure in our practice

  • Consultation & Diagnostics: Oldag will discuss your symptoms, possible triggers, pre-existing conditions, and your expectations with you. If necessary, a simple sweat test will be performed to determine the affected area.
  • Information & Planning: We will explain the mechanism of action, the possible effects, the treatment procedure, and the expected duration. Questions regarding tolerability or repeat treatments will be discussed in detail.
  • Injection: Dr. med. Gunther Oldag botulinum toxin superficially into the sweaty area. Depending on the region, approximately 10 to 20 small injections per side are necessary. The treatment takes about 20 to 30 minutes.
  • Aftercare: You can resume your normal social activities immediately after botulinum toxin treatment. Avoid saunas, sports, and intense heat for 48 hours. Mild redness or swelling is rare and subsides quickly.

How long does the effect last?

A noticeable reduction in sweat production typically appears a few days after treatment, usually around day 4. The maximum effect is reached after about 4 weeks. The duration of the effect is approximately 4 months, but this can vary individually depending on sweat gland activity, the area being treated, and metabolism. The treatment can then be repeated if necessary.

The treatment is not permanent, as the effects of botulinum toxin diminish over time. However, with regular use, the effect may stabilize.

What possibilities does therapy with botulinum toxin offer?

  • Fast effect: Initial results are usually noticeable a few days after the injection – sweating decreases significantly in many patients.
  • Precisely targeted: Only the affected glands are treated – no impairment of other bodily functions.
  • No downtime: You can immediately return to work and social life.
  • Reversible: The effect is temporary; repetition is possible, but not mandatory.
  • Psychologically relieving: Many sufferers report increased self-confidence and quality of life.

Are there any risks or side effects?

Treatment with botulinum toxin is very well tolerated when the correct dosage is used. In rare cases, it can lead to:

  • slight redness or small bruises at the injection sites,
  • a temporary feeling of tension or
  • a short-term weakness may occur during treatment of the hands.

Serious side effects are extremely rare. It is important that the treatment is performed exclusively by an experienced doctor – like at our practice Schoenheits-Hand-Werk.

Is the result immediately visible?

No. The effect begins after about 4 days and is fully developed after 4 weeks.

Can I do sports after the treatment?

Not exactly. For 48 hours, you should avoid strenuous physical activity or intense heat such as saunas or tanning beds.

Is the treatment painful?

The injections are generally well tolerated. A local anesthetic gel can be applied upon request.

Is sweating increasing in other parts of the body?

No. The reduction is only localized to the treated area. A so-called compensatory effect is extremely rare with botulinum toxin therapy.

Do I need to repeat the treatment regularly?

Not necessarily. Repeating the treatment is only advisable if the effect diminishes and there is still a need. The decision is made on a case-by-case basis.
